How to Upload Your Vaccination Card into Apricity

To upload your vaccination card into Apricity, please follow the instructions below. You may upload multiple vaccination cards.

Upload via Website

1. Log in to https://wcui.apricity-health.com/.
2. In the left navigation menu, click the Vaccinations button.
3. On the top right, click the + button.
4. In the Vaccination Details prompt, fill out the fields listed below.
   • Vaccine: Select the type of vaccination.
   • Date: Select the date of vaccination.
   • Dose: Select the type of dose. (1, 2, booster)
5. In the Vaccination Details prompt, click the upload file icon (it's very large). Select your vaccination file.
6. Once all the above is completed, click the Add button.

Upload via Mobile App

1. Log in to https://wcui.apricity-health.com/.
2. In the top-left navigation menu, click the Vaccinations button.
3. Click the + button.
4. In the Vaccination Details prompt, fill out the fields listed below.
   • Vaccine: Select the type of vaccination.
   • Date: Select the date of vaccination.
   • Dose: Select the type of dose. (1, 2, booster)
5. In the Vaccination Details prompt, click the upload file icon (it's very large). Select your vaccination file.
6. Once all the above is completed, click the Add button.
